Release form for Arkansas Crime Lab

This is a document needed when an individual is being examined by the Arkansas State Medical Examiner and for our company to be able to have them released to us.

Arkansas Crime Victim's Application

This document is for the reimbursement from the Crime Victim's Fund in which the deceased died from a violent crime.

Cremation Authorization

This is our cremation authorization for cremation in our Ascension Crematory.  This should be completed only by the IMMEDIATE NEXT OF KIN or the MAJORITY when there are multiple parties with the same level of authority as next of kin.

Cremation Self Authorization

This document, when executed properly, can be used by the crematory in lieu of our regular cremation authorization.  It is to be completed by the person to be cremated, witnessed by 2 individuals and notarized.  This is helpful when a person does not have an immediate next of kin to handle arrangements.

This provides the general directions for individuals who desire to mail the cremated remains of someone. 

Arkansas Right of Disposition Act of 2009

This is the A.C.A. § 20-17-102 Arkansas Right of Disposition Act which outlines who has the right to control disposition of a body in the state of Arkansas.  We must follow these guidelines when arranging cremation services to make sure there is no opposition by another party who has equal or higher rights to control disposition of an individual.

This is the forms to request military records for a veteran, to be completed by the next of kin.

Application for a US Burial Flag

Once completed this document can be submitted to a local post office to receive an honorable veteran's flag at the time of death.

VA Monument Application

This document is to request a marker for a veteran's grave.  You must have the form DD-214 to submit with the application when requesting.
